Job Title:      Family Case Manager

Minimum Qualifications

A Registered Nurse OR a clinical social worker with 1 year of experience in providing direct services to families with young children in a professional setting OR a master’s degree or bachelor’s degree in a behavioral science, social science, or health-related area OR a bachelor’s degree in any other area and one year of experience in providing direct professional child, family, or community services.

Performs case management duties and services at the Macon County Health Department (MCHD). Must establish and maintain positive relationships with participants, doctors, the general public, and other professional personnel. Must work with diverse community groups. Must be physically able to get in and out of a car many times per day in all types of weather and be able to adapt in a positive manner to various client living conditions. Must be physically able to assist with weights and measurements of adults and children many times per day. Must be able to carry, pull, and manage a minimum of 30 pounds of equipment daily. Must have a dependable vehicle for use during work hours, valid Illinois Driver’s License, and appropriate auto insurance.

Salary: $49,600
