Job Title:      Better Birth Outcomes Case Manager – Part Time

Minimum Qualifications

Registered Nurse or Masters of Social Work (MSW); Masters in Counseling Psychology, Licensed Professional Counselor (LPC); Licensed Clinical Professional Counselor (LCPC), Masters in Human Services and Counseling with specialization in counseling, Masters in Public Health with specialization in public health nursing, women’s health, or reproductive health. R.N. must submit proof of 20 Continuing Education Units (CEUs) every license period; copy of license; and copy of CPR/AED card. MSW must submit proof of CEUs and copy of license every license period. Performs Better Birth Outcomes case management duties with integrated Women, Infants, and Children (WIC) services at the Macon County Health Department (MCHD). Must possess the ability to establish and maintain positive relationships with participants, providers, the public, and other professional personnel. Must have the ability to work with diverse community groups. Must have a dependable vehicle for use during work hours, valid Illinois Driver’s License, and appropriate auto insurance. Must be physically able to get in and out of car many times per day in all types of weather and be able to adapt in a positive manner to various client living conditions. Must be able to carry, pull, and manage a minimum of 30 pounds of equipment daily.

Salary: $27.25/hr
